2. Information We Collect Through This Website
Quote requests and communications
The website quote form asks for contact information and basic details needed to understand and respond to your request. Depending on the form, this may include:
- Name, business or household name, email address, and phone number
- State and type of business or coverage requested
- Limited operational details, such as employee, vehicle, property, or coverage information
- Optional notes you choose to provide
- Source-page information that tells us which BLIS page or coverage topic led to the request
Emails and other communications may contain the information you choose to include. Do not put Social Security numbers, driver’s license numbers, payment-card information, medical information, or other highly sensitive information in a website note or ordinary email. If additional information is needed later, BLIS will identify an appropriate collection method.
Information that may be requested later
If you authorize BLIS to pursue, place, or service coverage, we or an insurance market may later request application and insurance-record information. Examples include addresses, payroll, employee and driver information, vehicle or property schedules, prior insurance, loss history, contracts, and tax identifiers when legitimately required. The public quote form is not intended to collect that complete application file.
Basic technical information
Our hosting and security systems may process an IP address, request date and time, requested page, referring page, browser or device information, and similar server-log data needed to deliver, secure, and troubleshoot the site. Section 6 describes the limited browser storage currently used by BLIS.
3. How We Use Information
We use website information to:
- Receive, review, and respond to your request
- Ask relevant follow-up questions
- Identify coverage topics or insurance markets that may be relevant when you ask us to do so
- Provide requested policy or certificate service
- Operate, secure, troubleshoot, and improve the website
- Prevent spam, abuse, and fraudulent submissions
- Maintain and honor cookie choices and privacy requests
- Comply with legal, regulatory, and recordkeeping obligations
BLIS does not use website quote details to build an unrelated advertising profile. Submitting a form does not guarantee a quote, price, approval, placement, or coverage.
4. How We Disclose Information
We may disclose information in these circumstances:
Website and form service providers
Vercel hosts and delivers this website. Zapier currently receives validated quote-form submissions so BLIS can route and respond to them. Email and other business-system providers may also process a request when needed to deliver it to BLIS. These providers process information for operational and service-delivery purposes.
Insurance services requested by you
If you ask BLIS to seek or arrange coverage, information may be disclosed to insurers, managing general agents, wholesale brokers, underwriters, inspection or claims providers, premium-finance or payment providers, and other insurance professionals involved in the requested transaction. BLIS does not send every website inquiry to every market.
Legal, safety, and business requirements
We may disclose information when reasonably necessary to comply with law, regulation, legal process, licensing or audit obligations; to protect the rights, safety, or security of BLIS or others; or in connection with a business transaction subject to appropriate safeguards.
BLIS does not sell personal information or share personal information for cross-context behavioral advertising. BLIS does not rent or trade website quote requests for third-party marketing.
5. Notice at Collection
At or before a quote-form submission, BLIS collects the following categories for the purposes and retention criteria described here:
- Identifiers and contact details: name, business or household name, email, phone, and state. Used to identify and respond to the requester.
- Commercial and insurance-request details: business type, coverage interest, limited operational details, and optional notes. Used to understand and review the request.
- Internet or network activity: source page and basic server-log information. Used to route the request and operate, secure, and troubleshoot the site.
These categories are disclosed to the website, form-delivery, email, and security providers needed to operate the request process. They may also be disclosed to insurance recipients if you ask BLIS to pursue insurance services. They are not sold or shared for cross-context behavioral advertising.
Website requests are kept only as long as reasonably necessary to respond, document the requested service, protect the site, and meet applicable legal or insurance-recordkeeping obligations. If a request becomes part of an application or client relationship, it may be kept with the applicable insurance record. Section 7 provides more detail.
6. Cookies, Browser Storage, and Tracking Choices
Current BLIS website storage
BLIS currently stores a first-party blis_consent record as a cookie and a matching local-storage record. It remembers your cookie choice and expires after no more than 395 days unless you update or clear it sooner. Necessary consent storage cannot be disabled while retaining a saved choice.
As of this policy’s effective date, BLIS has not configured a third-party analytics tag, advertising pixel, or cross-site behavioral advertising tag on this site. The Cookie Settings panel includes optional categories so visitors retain control if an optional tool is activated. A material change to those tools or purposes will require an updated notice version and, when required, a new choice.
Managing a choice
Use Cookie Settings in the footer to accept, reject, or change optional categories. Rejecting non-essential categories leaves them off. No optional tag is permitted to load before the corresponding category is allowed.
Global Privacy Control
When a supported browser sends a Global Privacy Control signal, this site treats it as an instruction to keep marketing storage and any sale or cross-context behavioral advertising sharing off. That signal overrides a previously saved marketing choice.
Do Not Track
Some browsers offer a separate “Do Not Track” setting. Because there is no consistently adopted industry standard for that signal, this site does not respond differently to it. BLIS does not currently operate cross-site behavioral tracking, and optional tools remain subject to the consent controls described above.
Other parties
We do not authorize third parties to collect personal information on this site for their own cross-site behavioral advertising. If you follow a link to another site, that site’s privacy practices and controls apply there.
7. Retention
- Consent choice: no more than 395 days from the most recent choice, unless cleared sooner.
- Website quote requests and correspondence: only as long as reasonably necessary to respond, document the requested service, prevent abuse, resolve disputes, and meet applicable legal and insurance-recordkeeping obligations.
- Hosting and security logs:for the operational and security period configured for the applicable service, then deleted or de-identified under that service’s retention process.
- Application and client records collected later: according to the policy lifecycle and applicable insurance, licensing, tax, legal, and recordkeeping requirements.
Retention may be extended when reasonably necessary for a legal hold, regulatory inquiry, security investigation, or dispute. BLIS will not retain personal information for longer than reasonably necessary for the disclosed purpose, subject to applicable law.
8. Security
BLIS uses reasonable administrative, technical, and organizational safeguards appropriate to the nature of the information, including encrypted transport for the public website and access controls for business systems. No transmission or storage method is completely secure, and BLIS cannot guarantee absolute security.
If you believe information submitted to BLIS has been compromised, contact service@blisins.com.
9. Privacy Rights and Requests
Depending on where you live, which law applies to BLIS, and whether an exemption covers particular insurance information, you may have a right to request access, correction, deletion, or a portable copy of personal information, or to appeal a denied request. You may also have rights concerning sale, targeted advertising, profiling, or sensitive personal information. BLIS does not currently sell personal information or use website information for targeted advertising or qualifying automated profiling.
California residents may have rights under the CCPA/CPRA when that law applies. Certain information processed under the California Insurance Code or federal Gramm-Leach-Bliley Act may be exempt from some CCPA provisions. Other state laws, including laws in Nevada, Texas, and Florida, have their own scope, thresholds, definitions, and exemptions. BLIS will evaluate each request under the law that applies to the requester and information.
To submit a request, email service@blisins.comwith the subject “Privacy Request” or call (818) 306-8333. Describe the request and state of residence. We may verify your identity and authority before responding. An authorized agent may submit a request when permitted by law. BLIS will not unlawfully discriminate against a person for exercising an applicable privacy right.
10. Children’s Privacy
This site is intended for business owners, household decision-makers, and their authorized representatives. BLIS does not knowingly collect personal information from children under 13 through this website. If you believe a child submitted information, contact service@blisins.com.
11. Changes to This Policy
We may update this policy as our practices or legal obligations change. The effective date at the top identifies the current version. For a material change, BLIS will post a reasonably prominent notice on the site and provide any additional notice or fresh consent required by law. Past policy versions may be requested using the contact information below.
